Cityblock Health, a value-based health care provider for Medicaid, dually eligible and lower-income Medicare beneficiaries, announced plans to locate operations in Indiana, creating up to 140 new high-wage jobs, including 100 during the first year alone, as part of a new partnership with MDwise.

Cityblock Health, which currently operates in seven states, will provide care for MDwise members in Indianapolis and Fort Wayne. This expansion will allow the company to build on its existing care service and deliver strong clinical programs for Medicaid members in Indiana.
Cityblock delivers medical care, behavioral health and social services to individuals from historically underserved and marginalized communities, particularly those with complex needs. This personalized care includes 24/7 access to multi-modal care delivery, with doctors and other clinical and non-clinical caregivers treating patients in their homes and community settings, via telehealth or at local Cityblock hubs. Cityblock’s members in existing markets have experienced significant reductions in emergency department visits and inpatient admissions, and overall improvements in engagement.

MDwise, a health maintenance organization serving more than 410,000 Hoosiers on Healthy Indiana Plan, Hoosier Healthwise and Medicare Advantage, will partner with Cityblock Health to bring comprehensive, personalized care to approximately 10,000 Hoosiers at launch.

The Indiana Economic Development Corporation committed an investment in Cityblock Health of up to $3.2 million in the form of incentive-based tax credits and up to $100,000 in conditional training grants based on the company’s job creation plans. These tax credits are performance-based, meaning the company is eligible to claim incentives once Hoosiers are hired and trained. The city of Indianapolis supports the project in partnership with Develop Indy.
